If you have moved outside of our catchment area then you will need to register with a new local GP Practice and you can update your contact details with your new GP practice. Visit the NHS Website to find a new local GP practice.
If you are still remaining within our catchment area and wish to change any of your contact details, we would prefer if you submitted the change via the Airmid app (see online services). If this is not possible, then please email us at email@example.com and include both your old and new contact details to highlight the changes.
